Situation
67 year old male, Living independently
Required no assistance,
Was working part-time
Admitted post hospital stay for sudden and extensive onset of Gillian Barre Syndrome.
Evaluation
Upon physical therapy evaluation, he
required 70% assistance getting in & out
of bed, 90% assistance with transfers
and was unable to ambulate.
His skilled PT and OT evaluations noted
extensive neurological weakness in his
pelvis and legs, overall deconditioning
and extremely poor balance. His
occupational therapy evaluation, he
required 5% assistance with upper body
bathing & dressing, 90% assistance with
lower body bathing & dressing, and 100%
assistance with toileting.
Treatment
Skilled PT focused on lower extremity
strength, balance, progressive ambulation
and stair training. Skilled OT focused on
upper body strengthening, dressing, bathing
and toileting.
Outcome
Upon conclusion of rehab stay, patient A was
able to return home at the following level: 0 %
assistance getting in & out of bed, 10% assistance
with transfers, ambulated 350’ with RW with 10%
assistance and able to perform the 17 stairs
needed to return home with 15% assistance; with
residual pelvic girdle weakness. Patient A was
functional enough to return home and continue
his physical rehabilitation at home. Upon his
rehab stay, patient A required 0% with bathing,
dressing and toileting.
A few months later, patient A visited Briarcliffe
Manor completely independent without an
assistive device and told the staff he was
planning a trip to Europe.
